Make an Appointment

If you are a current DTC patient, please call 817-263-0007 to schedule your appointment. Please note, if you have not been seen in over 3 years, you will be considered a “new patient” and will need to recomplete your new patient paperwork at your next visit. Regardless of this new patient status, you will be scheduled with the same physician that you have seen historically. Please note, follow up appointments are extended a 15 minute “grace period” for your appointment arrival time. However, if you arrive 15 minutes or after your scheduled appointment time, you will be asked to be rescheduled.

We also have the NextGen Patient Portal. This allows you to obtain lab results, request prescription refills and communicate directly with your provider. If you have not been set up for the patient portal, please call our office and obtain an enrollment token number. We invite you to use the patient portal for communication as it is directly between you and your provider.
